Proposed Service

Over a number of years, people have created separate diabetes centres which serve as a base for the diabetes team offering facilities both for the medical management of patients and also acting as an education and information resource centre. The creation of such a centre gives a heart to the service and allows a sense of ownership and belonging to be created for patients and carers, which is not labelled as a GP or hospital facility.

The treatment of patients with diabetes not only requires an active medical model but also the creation of an environment and atmosphere of education, true empowerment and empathy. Medical services must be easily and rapidly accessible so that problems, queries and concerns can be addressed quickly and promptly by appropriately qualified and trusted personnel.

Such aspirations for treatment must also recognise the prediction of the growing number of people who may have this disease and develop strategies to care and support a large numbers of patients and carers. The model of care we are looking to provide has been developed following national guidelines and is based on the strategies for chronic disease management of three Primary Care Trusts: North Surrey, Surrey Heath & Woking and Hounslow. The Stephanie Marks Centre will be the heart and focus of the service offering:
  • Treatment – a cutting-edge clinical service will allow access to advice and treatment, such as; routine annual screening, diabetes mgmt, podiatry, retinal screening, consultant referral, specialist nursing and dietetic services

  • Education – provision of services for patients to learn injection and blood sugar monitoring techniques. Healthcare professionals can learn how to streamline diabetes care in their practices. A resource will also be provided for ongoing education (DAFNE and DESMOND) and diabetes management.

  • Research – The centre will be designed to accommodate healthcare professionals and pharmaceutical companies by providing; nurse infrastructure for performance research, recorded information, patient communities and facilities to study

The clinical service provided would be ’cutting edge’ allowing a full day/direct access with advice and treatment. The diabetes doctor staff will be expanded to include Consultant specialists in diabetes, and GP colleagues with a specialist interest in diabetes will provide a high degree of doctor involvement in patient care. Equally important, will be the continued development of a range of diabetes nurse staff. Diabetes nurses have been shown to be extremely important in providing ongoing care of patients with diabetes both in the community and hospital settings.
